While our primary focus will be shifting from individual medical management to group support through the Acts2Thrive Community in 2025, we will still provide one-on-one medical services.

Comprehensive, Integrative Holistic Work-Ups for:

What’s Involved in a Work-Up?
Typically, there are 2-3 appointments with a provider in addition to medical testing to “work up” a suspected diagnosis based on the symptoms you are experiencing.
Initial Appointment: At this intake visit, Sara Arbogast, NP will discuss your concerns and take a thorough history. Labs and tests will likely be ordered at the end of this initial evaluation.
Testing & Physical Exam: This may include bloodwork, an ultrasound, and/or a pelvic exam.
Follow-Up Appointment: You will have a final appointment with Sara Arbogast where she will:

Explain your lab results

Explain your ultrasound results (if applicable)

Explain the implications of your pelvic exam (if applicable)

Discuss your diagnosis
Treatment/Management Options: Once you have a working diagnosis, we will provide resources on treatment and management options—whether that be pursuing integrative holistic treatment and ongoing management at PHA by joining our membership or by assembling your own care team elsewhere.

Member-Exclusive Services
The Acts2Thrive (A2T) Community Membership itself does not include any one-on-one medical care.
However, one must be an A2T Member in order to have access to treatment and ongoing medical management at PHA.
A2T Members may schedule for the following services, as appointment slots are available on a first-come, first-served basis:

Preventative Women’s Healthcare (routine annual exams and preventative care)

Endometriosis treatment, surgery, and ongoing monitoring

Fertility treatment (including fertility-enhancing medication & intrauterine insemination)

Polycystic Ovarian Syndrome (PCOS) Management

FAQs
Do you take insurance?
We are terminating all insurance contracts in 2025 and will no longer bill insurance after the date that your insurance company confirms that our contract with them has been terminated (“termination date”).
Visits will still be billed to insurance until your specific termination date. After termination, all medical care provided to both members and non-members will be fee-for-service. Click here for details.
What is the out-of-pocket cost for these appointments?
Once our insurance contracts have been terminated, our new prices will be as follows:

New Patient Intake- $400

Follow-Up Visit- $250

Gynecologic Annual Exam- $250

Gynecologic Ultrasound- $300

Are you a concierge practice?
No. Concierge medicine is grounded in delivering one-on-one patient care. Patients will typically pay a membership fee in exchange for 24/7 communication access to their medical provider(s), same-day/as-needed appointments, and longer appointment times.
Our membership is group-based and does not include individual care. Instead, members have exclusive access to scheduling appointments for treatment and ongoing management (at the standard fee-for-service rate), whereas non-members are only eligible to receive work-ups.
